Library Cards - SchoolsAll public and private schools located within the Algonquin Area Public Library District are eligible for a School Library Card. Each institution may have one library card. Each school must present to library staff a request for a school library card on the school’s stationery in the following format: The ______________________________ requests an Algonquin Area Public Library District School Library Card. The ______________________ assumes full responsibility for all materials borrowed and all fines incurred. I agree to observe all the rules and regulations of the Library.
Each school library card will be issued to the school representative who delivers the official request to the library, or directly mailed to the school Principal. School Library Cards are valid for one year from the date of issue. Approved |
