Lost Card Policy

All lost library cards must be immediately reported to the library.  Replacement cards must be obtained in person.  Payment must be made at the time the replacement card is issued.  The following rates apply to all lost cards:

1st lost card -	no charge
2nd lost card -	$2.00
3rd lost card -	$3.00
4th lost card -	$4.00
5th lost card -	$5.00
and beyond -	$5.00

To obtain a replacement library card each adult (16 years and older) must provide a photo identification card.

For children ages 5-15 a parent or guardian must sign the library card in the presence of a staff member.  The parent or guardian must provide a photo identification card.
